TIPS FOR PRACTITIONER
  FOREIGN BODY NOSE IN CHILDREN
 

Foreign Body (FB) nose in children is one of the commonest FB encountered by General Practitioners and specialist alike. The following are the salient features to be kept in mind:

  1. Don't panic: FB nose unlike FB ear or throat is not an emergency, infact nasal FB is often reported late by parents themselves.
  2. Axioms in FB Nose: One of the constant questions asked in MCQ based PG entrance exams is “cause of unilateral nasal discharge”, in children. The answer is always a Foreign Body Nose.
  3. Anatomy of Nose: It has to be remembered the nasal cavity is not a pair of tubes as it may look like, but a triangular space where you see only the bottom portion and mostly the FB gets stuck in this portion only. So a FB hook has to be passed above and behind circumventing the FB and pulling it forward rather the landing on the FB straight and pushing it backwards.
  4. You hold the child and I take the FB: You have to strike a deal with the parent that if they hold the child firmly, you will take the FB. The position is self explanatory in the picture.
  5. Pre Warn the Parent:
    1. You should prepare the parent about some blood letting since the Little's area is often in proximity of FB and old vegetable FB's invariably would have developed granulations, otherwise the parent may get shocked seeing the blood and become uncooperative themselves.
    2. You should also prepare them about inability to remove or in complete removal, it may so happen.
    3. Warn about slipped FB Rarely in FB like camphor piece which may melt and slip backwards into the throat In an alert child with good reflex the FB is swallowed in Toto. (see the answer to last month's quiz in monthly quiz section this issue)
  6. Innocent inferior Turbinate: The anterior end of inferior Turbinate is an erectile tissue which reacts to every stimulus, is often mistaken for a FB Nose or Nasal Polyp.
  7. Removal Under General Anesthesia (GA): In our experience, approximately, only one in every 100 FB Nose has to be removed under GA. Like the one in photograph An open safety pin with its sharp tip anchored to the floor and doing a see-saw movement could be removed only under GA with the help of nasal endoscope.
  8. Common FB's Nose: Erasers, Buttons, Peanuts, Tamarind seeds, Sticker bindi, etc.
